Outstanding Board of Health Award

The Wisconsin Association of Local Health Departments and Boards (WALHDAB) is currently seeking nominations for an Outstanding Board of Health. The purpose of this award is to recognize a Board that has made a significant contribution to public health at either the local, state or national level over a prolonged period. This award will be presented at the WPHA-WALHDAB Annual Conference, May 22-24th at the KI Convention Center in Green Bay, WI.


1.     Must be a member of WALHDAB.

2.     The board’s leadership in public health must be clearly evident.

3.     The accomplishment must either have documented results or be currently producing intended results.

4.     The activity may be accomplished through the health department, other agencies, or groups.

5.     One award is given each year.

6.     Nominations are only accepted from WALHDAB members.

To submit your application:

1.     Complete the short nomination form that is attached.

2.     Submit the application before Friday, April 6.
