Communicate Efforts and Findings

Dissemination of the final community health improvement plan to stakeholders and the community at large are requirements for public health accreditation and for non-profit hospitals. But, beyond that, ongoing and regular communication about the community health improvement process will help to keep key stakeholders and the public actively engaged. An overall communication plan for the project should include regular updates both during the planning process (to engage the community in the decision making) and during the implementation period (to provide progress reports on the impact of community health improvement efforts).
