Mid-Term Legislative Priority: Secure Reimbursement for Prevention Services

The Initial business case for coordinated team-based care was written to provide evidence and present a convincing argument for changing the manner in which healthcare is delivered and financed in Wisconsin. The below documents capture what we know today about this fluid and dynamic system, and proposes a solution to the issues and inefficiencies identified. The evidence throughout the initial business case demonstrates that a coordinated team-based approach is a cost-effective way to improve health outcomes. We can no longer work in silos and expect different results. We need to work together in a coordinated effort to truly impact the way we deliver care to patients and improve health outcomes in Wisconsin. 

A Coordinated Team Based Care Coalition has been formed to continue the discussion and update the constantly changing business case, and secure value based financing for coordinated team based care. The coalition will meet the first Thursday of each month from 9:00 a.m. – 11:00 a.m. in Milwaukee/Madison with teleconferencing available.
