Wisconsin Coffective Community Partnership Initiative

Wisconsin WIC is kicking off a new statewide initiative focused on coordinating efforts and strengthening community partnerships… and we don’t want your health department to miss out!

Your voice matters. As an influential part of the community, your health department’s perspective is a critical part of a new statewide initiative that focuses on coordinating efforts and strengthening community partnerships. With this initiative, the initial focus is on improving coordination around breastfeeding as a specific topic. However, the relationships built through this initiative can pave the way to shared resources, services, and consistent messaging that benefit the community and agencies involved, with any topic or focus area. Community partners involved in this initiative include: WIC, Hospital Clinical Leadership, Hospital Community Benefits, Local Health Departments, Outpatient Clinics, Home Visiting, Local Breastfeeding Coalitions, and Community Groups.

WHAT’S IN IT FOR YOU?

As a result of participating in the project, you will…

  • Better understand what others in your community are doing and their interest in partnering.
  • Receive a customized community match report that provides contact information for partners in your community, as well as shared interest in relationship-building activities with tools to help drive interest in building or strengthening partnerships to action.
  • Learn about concrete activities for relationship-building, as well as tools and resources to help.
  • Learn ways in which your local hospital’s community benefits program is interested in aligning their health priorities with yours.

The first phase of the initiative is to learn from you so your information can be included in the community match reports. Two surveys are provided with two different focuses. As the health officer, you may be able to respond to both surveys. However, we understand that in some local agencies, it might be more appropriate for you to forward one or both of the surveys to your staff who lead these efforts in your community. Please use your judgment to respond accordingly.

1.  CHIP/CHA Survey: This survey gathers information about the CHIP/CHA process within your community.

2. Public Health Breastfeeding: This survey gathers information about public health breastfeeding efforts (outside of WIC breastfeeding services).
