This section contains all of the resources created by the CHIPP Infrastructure Improvement Project referenced throughout the "Resources by Stage” sections, as well as some of the tools created by the Community Health Improvement in Action project (see CHIA Resources page for others). The resources below are intended to provide you with downloadable, customizable templates to support you throughout your community health improvement cycle. Note that some resources can be used at more than one stage, and that stages are color-coded for easy navigation. To open a resource, click on the Resource Name in the table below.

Strategy Map for Preventing and Reducing Alcohol Misuse 
An online, multi-layered visual guide to the community health improvement process with comprehensive links to resources, the Strategy Map focuses on alcohol misuse, but can be used for other health focus areas, especially as a template for designing a community logic model and evaluating efforts.

Sample Dashboard for Measuring the Progress and Impact of Alcohol-Related Initiatives
A template for organizing, tracking, and presenting data in a way that maximizes viewer understanding.  While the dashboard is alcohol-focused, it is a template that can be used for any health focus area and with any indicators. 
Enter your local information in the data source tabs, and the Dashboard takes care of the rest, creating tables and charts that report on your community's health.
